Memo: Daily Temperature and Symptom Reporting
Attention: All Fox Enterprises Employees
Due to recent changes in COVID-19 within our culture of Fox Enterprises. Each Foreman will be provided a laser thermometer. With this thermometer we will perform daily temperature checks of each and every employee of Fox Enterprises before the workday begins. The Foreman will record the temperature and ask each employee the questions on the attached chart.
If an employee presents a temperature of 100.4 degrees or higher it is the Fox Way that this employee will be sent home for the day and not to return until they are temperature and symptom free. Symptoms to look for include but are not limited to:
- Elevated temperature of fever of 100.4 or higher
- Shortness of breath and/or difficulty breathing
- Loss of smell of taste
- Persistent headaches
- Vomiting or diarrhea
Please notify your Project Manager IMMEDIATELY if you or a crew member are experiencing any of the above.
Also, effect immediately. NO more than 4 Fox Enterprises employees in a Fox truck at a time. We ask that you wear a mask when riding and ride with the windows down at all times.

ACCIDENT RESPONSE PROCEDURE
- Administer first aid immediately if required.
- Contact your foreman or project manager.
- Foremen or PM’s should complete the Accident Report & Investigation Form. Employees can complete the Injury / Illness / Near-Miss Report.
Time off requests must be submitted 5 days prior to the first day off.
